Requests for Information

From time to time, past students, families and legal entities require copies of documents and information held by Townsville Catholic Education (TCE). TCE is bound by various legislation (e.g. Privacy Act 1988 (Cth)) in relation to the privacy of the information it holds.  The following links will guide you to the correct ways to request information from TCE or our schools/colleges.

Please also note that our records are kept amongst numerous systems and locations, so allow 30 days for your request to be processed.

Are you a:

Current student or parent/guardian of current student

Requests for information about a current student from a parent, guardian, carer or independent student should be directed to the principal in writing via the school email address.

Past student or parent/guardian of past student?

This information applies to students that no longer attend the school or are the parents/guardians of a student that no longer attends the school.

A. If the past student is still a minor child the parent or guardian must:

  • Make a request in writing to inforequests@tsv.catholic.edu.au. You must provide the following information:
    • Full name and Date of Birth of the student.
    • The reason you are requesting the information.
    • A list of the records you require. Please be specific, e.g. school reports, medical records, enrolment forms.
    • The name of the school (including suburb) and the years attended (e.g. Grades 9-10, 2010 – 2011).
  • Provide verification of identity. This must consist of a copy of identification to be sighted and signed (certified) by one of the following prescribed persons:
    • Justice of the Peace, Commissioner for Declarations or notary public under Queensland or Commonwealth law
    • Lawyer
    • Conveyancer, or another person authorised to administer an oath, under Queensland or Commonwealth law.
  • If your name has changed and does not match the records the school holds you will need to provide evidence of your change of name (e.g. marriage/divorce certificate, change of name certificate).

B. If the past student is an adult the student must:

  • Make a request in writing to inforequests@tsv.catholic.edu.au. You must provide the following information:
    • Full name and Date of Birth.
    • The reason you are requesting the information.
    • A list of the records you require. Please be specific, e.g. school reports, medical records, enrolment forms.
    • The name of the school (including suburb) and the years attended (e.g. Grades 9-10, 2010 – 2011).
  • Provide verification of identity. This must consist of a copy of identification to be sighted and signed (certified) by one of the following prescribed persons:
    • Justice of the Peace, Commissioner for Declarations or notary public under Queensland or Commonwealth law
    • Lawyer
    • Conveyancer, or another person authorised to administer an oath, under Queensland or Commonwealth law.
  • If your name has changed and does not match the records the school holds you will need to provide evidence of your change of name (e.g. marriage/divorce certificate, change of name certificate).

IMPORTANT

Records cannot be provided to a parent requesting information on behalf of an adult student. The information belongs to the student and cannot be released without their consent. We require a written request from the student asking for the information along with verification of identification (see point B).

If the adult student does not have capacity to consent and you are their legal guardian, we require evidence of guardianship and verification of your identity (see point A).

A search for the records will not commence until we have received the verification of identity.

Third Party requesting information on current/past student or current/past staff?

This can include request such as:

  • Insurance requests
  • Subpoenas
  • Search warrants
  • Requests under Child Protection Act
  • Requests from the coroner or police
  • Requests from legal practices, lawyers and solicitors
  • Requests from medical services (mental health practitioners, psychologists)
  • Requests for CCTV or other image capture.

ALL Third-Party requests for information must be sent to either:

  1. Inforequests@tsv.catholic.edu.au for general requests, current or past student requests.
  2. spreports@tsv.catholic.edu.au for child protection, police, coroner or law enforcement requests.
  3. hrenquiries@tsv.catholic.edu.au for current or past staff or employment records request.

Please note:

  • verification of identity (VOI) of the person subject to the request may be required before we can process your request. This must consist of a copy of identification to be sighted and signed (certified) by one of the following prescribed persons:
    • Justice of the Peace, Commissioner for Declarations or notary public under Queensland or Commonwealth law
    • Lawyer
    • Conveyancer, or another person authorised to administer an oath, under Queensland or Commonwealth law.
  • If your name has changed and does not match the records the school holds you will need to provide evidence of your change of name (e.g. marriage/divorce certificate, change of name certificate)
Current or past staff member requesting staff or employment information?

Requests for information from a current or past staff member should be directed to the TCE People & Safety Team in writing to hrenrquiries@tsv.catholic.edu.au.

A past student or parent/guardian of a past student requesting information but living overseas?

If you are a past student or parent of a past student but are currently living overseas you will still need to provide verified copies of your identity. Each country has its own verification process, and we will accept identification documents verified according to the respective country’s requirements.

Researching Family History or School Reunion Information

If you are researching family history or would like to request information for a school reunion please make the request in writing to inforequests@tsv.catholic.edu.au.

Please note:

  • these searches may take longer to address as they are not our primary focus.
  • verification of your identity may be required before we can process your request. This must consist of a copy of identification to be sighted and signed (certified) by one of the following prescribed persons:
    • Justice of the Peace, Commissioner for Declarations or notary public under Queensland or Commonwealth law
    • Lawyer
    • Conveyancer, or another person authorised to administer an oath, under Queensland or Commonwealth law.

  • we cannot provide family history information if it contravenes the Privacy Act 1988 (Cth).

Replacement Year 12 Certificates

Not all schools retain a copy of year 12 certificates. Queensland Curriculum and Assessment Authority (QCAA) can provide replacement copies of Year 12 certificates – see HERE for more information. 

Where to find a Justice of the Peace (JP)

JPs provide a free service and are based in shopping centres. See HERE for their locations. 

Parent or guardian - must be listed on the student’s enrolment forms or birth certificate to be able to access information and records about the student (when they are a minor child).

Family, friends, informal carers – cannot access any student information without the consent of the owner of the records (either a parent/guardian or the student).
